We are dedicated to caring for our patients as we would for our own families and loved ones. Therefore, we all have a responsibility to report any potential violations of our Code, our policies, or the law. Employees seek guidance, raise concerns, or report suspected misconduct in faith are upholding our code and supporting our mission. Merigold Hospice and its network of healthcare providers do not tolerate any form of retaliation against those who issues or concerns sincerely. Reporting “in good faith” means being sincere your attempt to provide honest and accurate information, even if the concern raised is later found to be incorrect.

Merigold Hospice, our Care Transition Program ensures timely access to the level of care. Our skilled clinicians focus on effective communication and coordination. Uncoordinated care can cost nearly $80,000 per.* Our program emphasizes a continuum of care to enhance outcomes and reduce costs, ensuring patients receive necessary support at home, minimizing readmissions, and assisting families. Meet Your Hospice Team
MEDICAL DIRECTOR / HOSPICE PHYSICIAN
Oversees the patient's care plan and manages pain and symptoms, coordinating with the attending physician as necessary.
HOSPICE VOLUNTEER
Offers companionship and assists with household tasks as needed.
SOCIAL WORKER
Addresses emotional and social needs while identifying community resources for families and may support end-of-life planning.
REGISTERED NURSE / CASE MANAGER
Delivers direct physical care to alleviate symptoms and ensure patient comfort, coordinating with the entire hospice team.
CNA – CERTIFIED NURSING ASSISTANT / HOSPICE AIDE Provides personal care services, including bathing and grooming.
CHAPLAIN / SPIRITUAL COUNSELOR / BEREAVEMENT COUNSELOR
Addresses the spiritual needs of patients and families upon request and provides bereavement support.
